Immigration Exams (I693)
& Green Card

A Physician who is designated civil surgeon for the Department of Homeland Security will be in charge of your USCIS (U.S. Citizenship and Immigration Service) examination.
The authorized Provider will conduct your medical examinations for individuals seeking an immigration benefit in the United States and complete Form I-693.

Requirements

  1. A Photo ID: U.S. Driver's License, Military ID, Passport, Green Card, or a State-issued ID.

  2. For form I-693, patients must fill out their portion in capital letters.

  3. All medical records, including all vaccinations with translation in English when applicable (Flu, MMR, Chicken Pox, Hepatitis B, TDAP, and boosters for many other vaccines if necessary). Please visit the Centers for Disease Control and Prevention for more information.

  4. Our civil surgeon will order labs and necessary tests, as we do not accept outside labs.

Payment

We accept Debit card, cash, or Zelle only. The original fee, $500, is for completing form I-693 - Report of Medical Examination and completing Vaccination Record only. The fee does not include labs, tests, vaccines, or treatments if necessary.

Additional tests, vaccinations, and treatments are not included in the initial price for the exam. It is the applicant's sole responsibility to pay for those other procedures.

What to expect at your appointment:

  • Completion of Form I-693 - Medical examination, Lab draws, Urine test, QFT blood draw to test for TB (If test results are positive you may need a CXR). If a chest X-ray is needed, you will be given a referral form to do it at an outside facility.

  • Review of Immunization Status - Most vaccinations can be given at the health department for an additional fee.

Follow Up visit in two weeks:

  • The civil surgeon will complete the form I-693 and seal it in an envelope. You should then forward the sealed envelope to the USCIS. Please do not open the envelope - USCIS may not accept an unsealed envelope. We will provide you with a copy.
